Application forms

The IOM AVR application forms below can be filled either electronically on the screen or printed and filled in by hand. However, forms filled on the screen must always be printed for the applicant to sign before the forms are forwarded to IOM.

Please have applicants fill in both an AVR application form and an application form for reintegration assistance. A questionnaire for the returnees is also included in the form packages. This can be filled and forwarded to IOM together with the forms. Even though it is not mandatory to fill the questionnaire, the information gathered through it is important for IOM for the future development of Assisted Voluntary Return practices in Finland. The questionnaire can be filled in anonymously, and the response will have no effect on the return arrangements. Note that IOM may have to ask for more forms to be filled, as for example if the person must fill an application for a travel document.

Assisted Voluntary Return - form packages

These forms cannot be used for Assisted Voluntary Returns of EU citizens who are not eligible to participate in the project. To apply for AVR of citizens of the EU, please use this form. Kindly note that the project does not support the return of EU citizens and that the referring partner (as for example a reception centre) can be asked to pay the travel costs of a voluntary return of an EU citizen with IOM.

Sending the forms to IOM Helsinki
If the application forms have been filled electronically, they must be printed and signed before being forwarded to IOM. Please return the filled and signed forms to IOM Helsinki by fax, scanned in an e-mail, or by surface mail.

If the applicant has documents from the country of origin (travel document, national ID etc), either in copy or original, kindly include copies of these documents with the application and inform if the person has the original documents or if the originals are somewhere else.

If the applicant prefers to come personally to the IOM Helsinki office and needs counselling in her/his own language, it is recommended to call in advance to make an appointment.
