If you have decided to return, maybe after talking to your social worker, you must fill in and sign two forms in order to apply:

These forms should be forwarded to IOM Helsinki together with any relevant attachements, as for example copies of your personal documents from your home country (if you have any).

The forms can be delivered directly to IOM, or you can send them by mail, by fax or scanned as e-mail attachments. Your social worker might help you with this.

What you need to include in your application
If you have travel documents, please deliver copies of them to IOM together with your application. If you don’t have a passport, IOM will help you to arrange a travel document.

Please include any information in your application that you think can be important for your travel, as for example if you have a serious medical condition.
